The Clinical Liaison RN (Home Infusion) supports home infusion pharmacy by building and maintaining referral relationships with healthcare providers and facilities. The role involves assessing patient suitability for home infusion therapy, facilitating care transitions, and providing clinical consultation and education to providers, staff, patients, and families. This position requires licensure as a registered nurse and experience in clinical roles, with a preference for nursing degrees and related sales or marketing experience in healthcare.

This position will be supporting Oregon Home Infusion Pharmacy.
The Home Infusion Clinical Liaison position is responsible for creating and maintaining positive referral relationships in order to increase Home Infusion Referrals within assigned accounts. This role includes working with discharge planners, facility staff, physicians, and other individuals to assess appropriateness home infusion, facilitate patient care transitions onto service and ensure there is appropriate documentation for ordered infusion therapy. Consultative functions include, but are not limited to: providing consultation and informational presentations regarding home infusion therapy, and serving as a clinical resource and educator for ordering providers, facility staff, patients, families on Home Infusion Therapy.
